Event Synopsis*: The "Manuscript Writing Made Easy" is a 4-week online course which aims to provide guidance to healthcare professionals on how to write a health-related manuscript based on the IMRAD (Introduction, Methodology, Results and Discussion) format. Autopsy findings of COVID-19:  Lesson learnt

 

Summary

      The new COVID-19 has caused more than 1.7 million deaths worldwide.  However, the pathogenesis and pathologic features of COVID-19 is only partially understood.

       Despite modern diagnostic tests, autopsy is still great importance and may be a key to understanding the biological characteristic of COVID-19 and pathogenesis of the disease.   A literature review of the present evidence on the autopsy examinations highlights the main pathologic features of the disease.

            Autopsy studies show a multisystemic pathology which leads to death typically in those with comorbidities. The lung findings are most significant for majority of those infected.   Generally, appear heavy and oedematous, microscopically show diffuse alveolar damage with hyaline membrane formation, alongside microthrombi in small pulmonary vessels.

Other organ systems are frequently involved including with widespread microscopic thromboses in numerous organs, as well as liver, kidneys, and lymph nodes.

There is also evidence that the new COVID-19 causes endothelial dysfunction which in turn could be responsible for multiorgan dysfunction. Endothelial dysfunction could be explained by the expression of the receptor binding domain on the COVID-19 surface, which binds ACE2 (angiotensin converting enzyme 2) receptors, localized in many human tissues including heart, kidneys, liver, brain, and the endothelium.

Autopsy findings in these most severely affected patients can serve as a valuable source of information in studying this new disease process and can influence the therapeutic strategies and ultimately reduce mortality.

Bipolar Mood Disorder in a Young Man with 48,XXYY Syndrome

 

48,XXYY syndrome is a rare condition. The presentations of impulsive and aggressive behavior have been reported in several case reports among patients with 48,XXYY syndrome. The management of the psychological impact of this condition is a significant issue faced by families, carers, and healthcare professionals. We report on a patient with underlying 48,XXYY syndrome with Attention Deficit Hyperactivity Disorder (ADHD) and intellectual disability, diagnosed later in adulthood with Bipolar Mood Disorder and benefited greatly after being started on Lithium. From the adult psychiatry perspective, we suggest that for a patient with 48,XXYY, routine psychological screening including the presence of mood disorder symptoms such as mania and depression, should be done by healthcare professionals. In view of higher risk of metabolic syndrome and deep vein thrombosis associated with 48,XXYY syndrome, they also should be screened for these conditions

Course Synopsis:

 

Trauma is an increasing course of morbidity and mortality. The Emergency Department is visited by many trauma cases on a daily basis.

This CME focuses on the many challenges facing the emergency residents and other health care professionals involved in the clinical care of trauma patients that attend to the emergency department. This CME will help to increase the knowledge and enhance the competence of attendees in handling trauma emergencies.

 

Objectives:

 

-     To understand what is Acute Trauma Care, and who comprises of the Trauma team

-     To know what is damage control resuscitation and when to activate the massive transfusion protocol

-     To recognize the common head and neck injuries that present to the emergency department and how to treat them

The Malaysian Hospice Council is commencing a continuous palliative care education programme later this month which will run for a year. This will follow the model of the ECHO project pioneered by Dr Sanjeev Arora from Alberquerque, New Mexico. The ECHO project has expanded from its original roots to now run in 44 countries across the world with a total of 423 hubs and 920 programmes operating internationally. The project aims to move knowledge and not patients and providers by utilising a low cost platform (Zoom) to provide accessibility to leverage scarce resources by using a 3G internet connection. In doing so, best practices and evidence based care can be promoted to reduce disparities in healthcare in different settings. The model uses case based learning to encourage peer led learning and master complexity where participants address learning needs that are directly relevant to them. By demonopolizing knowledge, all can teach and all can learn and learning can be amplified to reach others rapidly. 

ECHO uses a hub and spoke model; hubs are the centre where the programme is run where content experts and multidisciplinary teams are based. Participants join as spokes to the hub. ECHO sessions last 1 hour and entail a series of 2-3 case discussions presented by selected spokes for collaborative input by all, as well as a 15-20 minute didactic teaching session by an expert. 

The MHC ECHO will be conducted every 2-3 weeks over the next year encompassing a total of 29 topics. Feedback and evaluations will be obtained after each session for purposes of quality improvement and to assess learning in participants.

Phase 1 of the project is aimed mainly towards hospice staff. Initially, we hope to begin with a maximum of 20-30 spokes. Phase 2 will launch later focussing mainly on Klinik Kesihatan staff and MOH/MOE hospital staff interested in improving their knowledge and skills in palliative care.

 A 2 hours and 15 minutes session where an unknown case from Department of Psychiatry is presented together with the results of relevant investigations (laboratory or imaging). The differential diagnosis and eventually the final diagnosis will be discussed and presented. After one year absence, the Annual Dialysis Meeting will be held at the Sheraton Petaling Jaya Hotel, Petaling Jaya, Selangor. The sixteenth in a series of Annual Dialysis Meetings organised by National Kidney Foundation of Malaysia since 2003 is being renamed the Annual Dialysis Conference with effect from 2020.

The  conference with the theme “Holistic Care in Dialysis” aims to include topics on Mental Issues in Haemodialysis, Palliative Care, Nutritional Issues and Sexual Dysfunction in dialysis. The topics for the scientific programme aim to bring to the fore that it is time to include other disciplines to give dialysis patients the chance to live longer, happier, and more productive lives with a treatment that fits better with those things they value in their homes, workplaces, and communities.

Patients on dialysis are in a situation of abject dependence on a machine, a procedure and a group of qualified medical professionals for the rest of their lives. A multidisciplinary team effort is often needed in the management of such patients.

Scope of the Workshop:
The workshop aims to help participants understand various forms of data and help develop analytical skills for processing it into useful information. Mode of learning will be mostly interactive, with maximum time dedicated to hands-on training using planned exercises.

Course Objectives:
At the end of the course, you will have:

  • An introduction to exploratory data analysis
  • Understanding of some common statistical tests and their interpretation
  • Experience of using SPSS for statistical data analysis

The learning objective of this online workshop (conducted over two-weeks with a 2-hour sessions for two days in a week) is to give each participant experience on how to undertake basic statistical tests using SPSS as the statistical software.
