-Parasite infestation is a very common problem with up to 25% of the world population suffering from it.

-Parasites often infect the gastrointestinal (GI) tract of humans.

-Entamoeba is a protozoan parasite found in the stool and has a high rate of infection and death.

-Giardia is the only example of a flagellate which causes a GI infection.

-Taeniasis is caused by tapeworm which can come from undercooked pork or beef.

-Hymenolepsis, the dwarf tapeworm, is the most common cestode parasite in humans.

-Ascaris is the most common parasite found in the human alimentary canal.

Event synopsis / Objective:

The National Intensive Course in Rehabilitation Medicine is a unique event organized by the Rehabilitation Medicine unit, Department of Orthopaedic and Traumatology, Faculty of Medicine, UKM in  collaboration with Department of Rehabilitation Medicine, Faculty of Medicine, Universiti Malaya and Malaysian Association of Rehabilitation Physician (MARP).  This course will be a 3 day course organized from 1st to 3rd October 2019 at Advanced Surgical Skills Centre, Hospital Canselor Tuanku Muhriz (HCTM), UKM. Wide range of rehabilitation topics such as spinal cord injury, paediatric, acquired brain injury, stroke, diabetic foot problems and amputee rehabilitation will be covered during the course. Professors/ senior lectures and consultant rehabilitation medicine physicians from Malaysian rehabilitation medicine fraternity are going to be actively involved in delivering their talks, conducting interactive sessions and case discussion.

Participants will have a great opportunity to be involved in interactive case discussion with the experienced speakers.

To learn normal ultrasound anatomy and scanning techniques for the chosen module.

Participants under supervision of the sonographer will learn the scan techniques and identify normal anatomy either on live models or phantoms.

Abdominal phantom 

Pathologies shown are liver cyst, liver tumours, gallstones, bile duct stones, kidney stones etc.

Pelvis female phantom

Pathologies shown are uterine fibroid, dermoid cyst, ectopic pregnancy, blood in Pouch of Douglas and endometrial cancer.

Acute abdomen / FAST phantom

Pathologies shown are acute cholecystitis, aneurysm, acute appendicitis and diverticulitis as well as free fluid or bleeding in perihepatic,  perisplenic, pelvis and pericardium.

Ante-natal phantom

A 23-week fetus is provided where participants will be able to scan and learn the correct techniques for fetal growth assessment.

How is our course difference from others?

1.       We are committed to maintain small groups of 2-3 participants only to allow  maximum hands-on opportunities.

2.       In addition to live model, we also use phantoms. Each phantom has an array of pathologies that the participants may scan and elicit the findings.

3.       NR is GP-based (as opposed to hospital based). As such, the kinds of pathologies that we see are much more relevant to GP practice.

4.       Each module will be conducted in 2 phases – workshop first followed by lecture sometimes later. In between, the participants may start scanning in their own practice. If they face any problems or uncertainties, they may bring   them up when we meet for the lecture.

Title :  Pregnancy and Renal Transplant

Madam LJY, a 28 year-old primigravida, was a post- renal transplant patient with chronic hypertension. She had a history of resolved cardiomyopathy secondary to uremia. Her pregnancy progressed well until 33 weeks, during which she had to be delivered due to worsening renal function. This case highlights the importance of multidisciplinary input in the management of pregnant woman with renal transplant, to ensure favourable maternal and fetal outcome.

GLYCATED HEMOGLOBIN (HBA1c): THE PERSPECTIVE OF CLINICAL BIOCHEMISTRY

SUMMARY

Glycated hemoglobin is a modified hemoglobin with a stable adduct of glucose to the β-chain. Diabetes mellitus is characterized by chronic hyperglycemia, and the measurement of glycated hemoglobin is an established means of monitoring glycemic control of diabetic patients. Measuring HbA1c is also recommended for the diagnosis of diabetes mellitus. Normal adults consist of about 6% of glycated hemoglobin which in turn is made up of HbA1a1, HbA1a2, HbA1b and HbA1c. HbA1c is the most abundant fraction and comprises about 5-6% of the total HbA. The glycated hemoglobin synthesis occurs slowly, continuously and irreversibly throughout the life span of RBC. Glucose in the open chain format binds to the N-terminal amino acid in a non-enzymatic process. Many reports have shown that the concentration of total glycated hemoglobin measured by commonly used methods may change significantly over a period of time. Hence, the physician must consider an overall patient profile before accepting an abnormal HbA1c value. This presentation will include an overview of the chemistry of glycated hemoglobin, and the potential advantages and limitations of HbA1c value for the diagnosis of diabetes mellitus.

Title : New approaches for the management of Diabetic Retinopathy

Diabetes mellitus (DM) has become a major global public health problem. One of the long-term microvascular complications of diabetes include Diabetic Retinopathy (DR). It is the commonest cause of visual loss among working adults in Malaysia. Within 20 years of diagnosis of DM, nearly all Type 1 DM patients and almost 2/3 Type 2 DM patients will have DR.

Visual impairment from DR can have a great impact on patient’s daily activities and reduced independence and productivity, decreased social interactions, difficulty in managing their diabetes care such as insulin injection, sugar monitoring, foot care etc.

The therapeutic approaches in DR management aim for early diagnosis and slowing down the progression of DR. It is important to have regular screenings, good blood glucose level and blood pressure control and healthy lifestyle. Surgical treatments include laser retinal photocoagulation and vitreoretinal surgery. Laser retinal photocoagulation reduces the risk of blindness caused by vitreous haemorrhage or retinal detachment. However, there are side effects of  laser therapy such as diminished vision field, reduced colour vision and night vision etc. Vitreoretinal surgery is only indicated for persistent vitreous haemorrhage and retinal detachment in advanced diabetic eye disease.

 

Existing treatment options for DR are limited. Many researches focus on understanding the molecular and biochemical mechanisms of  DR development and progression.  It has been noted that Vascular Endothelial Growth Factor (VEGF) level is increased in DR. VEGF plays an important role in neovascularisation and breakdown of the blood-retinal barrier.  Therefore, strategies to block VEGF or its activity in the eye provide promising DR treatment. Intravitreal Anti-VEGF injections therapies for eg. Ranibizumab and Aflibercept are now used for treating diabetic retinopathy with diabetic maculopathy with evidence of improvement in patient's vision. 

 

There are also typical features of  inflammation such as increased levels of vitreous cytokines, chemokines and growth factors. These lead to vascular leakage and haemorrhage. Hence, another approach of DR treatment is to target inflammation. Corticosteroids are injected intravitreally and some comes in the form of extended release corticosteroids for long term treatment of DME. Similarly, corticosteroids has been shown to slow down DR progress.

The objective of this research is to extend the core technologies of exiting clinical decision support systems (CDSS) infrastructure towards knowledge evolution and maintenance utilizing diverse clinical resources such as unstructured clinical notes, guidelines, images, and structured EMR/EHR data. This innovative idea aims to design and develop evidence and dialogue-based clinical decision support system to assist physicians in decision making during patient care. We propose dialogue-based Intelligent Medical Platform (IMP) that provides services from up-to-date evolutionary knowledge bases. The knowledge base is created using medical standards, which makes the CDSS integration easier with legacy clinical systems. The system is equipped with a dialogue environment in the form of text, voice, and image to physicians and patients during treatment, diagnosis, and health-related activities. It overcomes the limitations of the existing system in terms of an evolutionary knowledge base, dialoguing ability of interaction, integration with legacy healthcare systems such as hospital information and management systems (HMIS), and handling multimodal data from diverse input sources.

“Atypical Presentation with Diagnostic Dilemma”

A 23-year-old Malay lady, a university student from east coast, previously well, presented with 3 days of diarrhea, vomiting and mild abdominal pain.  She had no fever and no other family members had similar symptoms except for her father which was resolved.  She denies eating outside.

Clinically she was overweight, comfortable and not toxic looking, afebrile with stable hemodynamically.  There was no peripheral lymphadenopathy palpable.  Neither there was hepatosplenomegaly nor could ascites be appreciated.  Other systems were unremarkable.

Initial laboratory investigations showed hypochromic microcytic anemia (Hb 10.0 g/dL), marked thrombocytosis (platelet 906 x 109/L, leukocytosis (25.1 x 109/L, Neutrophil 81.7%), alkaline phosphatase 275 U/L, AST 139 U/L, gamma GT 343 U/L, serum albumin 29 g/L, serum uric acid 0.53 mmol/L and normal renal function.  The cancer markers shows raised CA 125 (388.7 U/mL).  Septic workout and stool for ova and cysts were negative.

She was treated as infective acute gastroenteritis until ultrasonography and CT scan of the abdomen reveals multiple lobulated mass in the liver and the right ovary with para-aortic lymphadenopathy seen suggestive of metastases.  Histopathologically reveals stage IV diffuse large B cell lymphoma (DLBCL).

The atypical presentation this young lady lead to a diagnostic dilemma which later end up with unfavorable diagnosis.

During the presentation, will discuss the final diagnosis.

Synopsis:

The objective of this study is to evolve the knowledge base for the diagnosis, treatment, and follow-up for different diseases such as Congestive Heart Failure (CHF), with the help of physicians’ experience and patient information in real practice. A team of expert cardiologists analyzed the clinical practice guidelines (CPG) for possible heart failure (HF) disease with different classification of ejection fraction such as heart failure mid-range ejection fraction (HFmrEF), heart failure reduced ejection fraction (HFrEF), heart failure preserved ejection fraction (HFpEF), and Not heart failure (Not-HF). Based on CPG and physicians’ experience, the knowledge is modelled using mind map. Knowledge engineers transformed mind map into decision tree model to identify and resolve the anomalies in the knowledge by intensive meetings with physicians. The verified decision tree is transformed into production rules (15409) using a user-friendly intelligent-knowledge authoring tool (I-KAT) for execution of knowledge by clinical decision support system (CDSS). The CDSS was tested on 600 patients of heart failure. In second phase, we trained machine learning algorithms such as Random Forest, classification and regression tree (CART), Decision Tree, Naïve Bayes, Deep learning) on the same dataset and finally compared and validated both of knowledge model. The hybrid knowledge acquisition methodology of cardiovascular CDSS is evaluated on 600 patient data and achieved accuracy of 98.3%. While separately, the highest accuracy of data-driven approach was achieved by CART algorithm, which was 88.5%, and the expert-driven approach accuracy was 90%.

Cardiac arrest is a treatable medical event that requires immediate action to save a life. Cardiac arrest can strike indiscriminately and without warning, often leaving grieving family members and friends to question what could have been done differently. 

When the heart stops, the brain will begin to die within 4 minutes. An ambulance takes 10-15 minutes to reach the victim. If CPR were instituted early, even before the arrival of the ambulance, it could make a difference to the victim’s survival. As cardiac (heart) arrest could occur at home, in the office, on the streets, bystanders able to perform CPR becomes valuable for the victim.

Institut Jantung Negara (IJN), being the premier cardiac hospital, have taken the first step of public responsibility by offering Basic Life Support (BLS) training. Basic Life Support (BLS) is the foundation for saving lives after cardiac arrest. It is designed for healthcare professionals as well as public to provide emergency care to patients in a wide variety of settings, and teaches both single-rescuer and team basic life support skills for application in pre-hospital and in-facility settings.

 Course Fee: RM 371.00 (including 6% SST)

Jabatan Oftalmologi Hospital Sultan Ismail Johor Bahru telah mula beroperasi sejak tahun 2006 . Dalam tempoh ini, jabatan ini telah menerima rujukan dari 7 hospital yang berhampiran iaitu Hospital Sultanah Aminah, Hospital Kluang, Hospital Pontian, Hospital Kota Tinggi, Hospital Mersing, Hospital Permai, Hospital Kulai serta Klinik-Klinik Kesihatan yang berkenaan. Dalam tempoh itu juga, jumlah pesakit yang menerima rawatan mata telah meningkat dari  15107 orang pada tahun 2013 kepada 28057 pada tahun 2018.

Berdasarkan data pada tahun 2018, 30% daripada jumlah kes yang dirujuk memerlukan rujukan segera manakala selebihnya adalah rujukan dengan temujanji dalam tempoh yang tertentu. Sebilangan besar kes (94%) adalah rujukan dari ‘primary care’ seperti klinik kesihatan dan klinik swasta. Namun, disebabkan kurang pendedahan mengenai penyakit mata, banyak kes kesilapan diagnosis berlaku dan ini menyebabkan tempoh temujanji yang diberikan tidak mematuhi KPI yang telah ditetapkan.

Bagi memastikan pesakit mendapat rawatan yang terbaik, Jabatan Oftalmologi HSI telah mengambil inisiatif untuk menganjurkan kursus “CPG Diabetic Retinopathy & patient’s management” bagi pegawai perubatan dan pegawai optometri serta paramedic di sektor awam dan sektor swasta bagi memastikan penyampaian perkhidmatan dapat diberikan kepada pesakit dengan lebih efisyen.

OBJEKTIF

1.   Meningkatkan pemahaman anggota kesihatan primer tentang ‘Diabetic Retinopthy’

2.   Memberikan tips dan kaedah dalam mengendalikan kes-kes ‘Diabetic Retinopthy’  yang memerlukan rujukan segera

Brief description based on the Event Title
The AOTrauma Course—Advanced Principles of Fracture Management builds upon the AO principles and techniques learned in the AOTrauma Course—Basic Principles of Fracture Management and focuses on the principles and techniques of operative fracture management in more complex injuries. This course is targeted at surgeons who are at the threshold of becoming independent surgeons and taking over decision-making responsibility for the treatment of complex injuries. Participants must have already completed the AOTrauma Course—Basic Principles of Fracture Management and must be actively involved in trauma management.

Online precourse self-assessment prepares participants for the course and allows the faculty to tailor the course to the needs of the participants. Before attending the course, participants are expected to complete an online module on fracture reduction. The course will be taught in a modular format. Each module consists of several evidence-based lectures, which will cover the key information required. In practical exercises participants will be trained in the application of various techniques. Discussing cases in small groups will help participants to understand decision-making and
management skills. After the course, an online postcourse self-assessment will provide participants an opportunity to review the important topics from the course. In order to support self-directed learning, a range of additional online resources will be offered.

The learning objectives are at the end of this course, participants will be able to: 
• Apply reduction techniques in fracture management with attention to soft tissues
• Assess and treat complex diaphyseal and (peri)articular fractures using advanced application techniques
• Demonstrate strategies for assessing and treating open fractures and soft-tissue injuries
• Initiate appropriate management for patients with pelvic injuries and polytrauma
• Recognize complications and manage them accordingly

Cardiac arrest is a treatable medical event that requires immediate action to save a life. Cardiac arrest can strike indiscriminately and without warning, often leaving grieving family members and friends to question what could have been done differently. 

When the heart stops, the brain will begin to die within 4 minutes. An ambulance takes 10-15 minutes to reach the victim. If CPR were instituted early, even before the arrival of the ambulance, it could make a difference to the victim’s survival. As cardiac (heart) arrest could occur at home, in the office, on the streets, bystanders able to perform CPR becomes valuable for the victim.

Institut Jantung Negara (IJN), being the premier cardiac hospital, have taken the first step of public responsibility by offering Basic Life Support (BLS) training. Basic Life Support (BLS) is the foundation for saving lives after cardiac arrest. It is designed for healthcare professionals as well as public to provide emergency care to patients in a wide variety of settings, and teaches both single-rescuer and team basic life support skills for application in pre-hospital and in-facility settings.

 Course Fee: RM 371.00 (including 6% SST)
