Keynote speakers and symposium presenters will challenge the audience and stimulate new ways of thinking about assessment of individuals and teams, and evaluation of programmes. The continuum of education will be addressed in medicine, dentistry, nursing and the other healthcare professions, from undergraduate through postgraduate/specialist training to the often neglected area of continuing professional development. 

On the basis that all educators should have an understanding of the principles of assessment and evaluation, Ottawa 2020 welcomes participants from around the world, whether assessment specialists or relatively new to the area, to share their views and present different cultural perspectives. APLS ( Advanced Paediatric Life Support ) course is an international course done by the MPRS under the Advanced Life support Group in United Kingdom. The regulations are strictly adhered to. We aim to train doctors dealing with critically ill children in handling pediatric emergencies. Drs working in Pediatrics, Emergencies, Paediatric Surgical, Neurosurgical and anaesthesia are the target group.

Cardiac arrest is a treatable medical event that requires immediate action to save a life. Cardiac arrest can strike indiscriminately and without warning, often leaving grieving family members and friends to question what could have been done differently. 

When the heart stops, the brain will begin to die within 4 minutes. An ambulance takes 10-15 minutes to reach the victim. If CPR were instituted early, even before the arrival of the ambulance, it could make a difference to the victim’s survival. As cardiac (heart) arrest could occur at home, in the office, on the streets, bystanders able to perform CPR becomes valuable for the victim.

Institut Jantung Negara (IJN), being the premier cardiac hospital, have taken the first step of public responsibility by offering Basic Life Support (BLS) training. Basic Life Support (BLS) is the foundation for saving lives after cardiac arrest. It is designed for healthcare professionals as well as public to provide emergency care to patients in a wide variety of settings, and teaches both single-rescuer and team basic life support skills for application in pre-hospital and in-facility settings.

How is our course difference from others?

1.       We are committed to maintain small groups of 2-3 participants only to allow  maximum hands-on opportunities.

2.       In addition to live model, we also use phantoms. Each phantom has an array of pathologies that the participants may scan and elicit the findings.

3.       NR is GP-based (as opposed to hospital based). As such, the kinds of pathologies that we see are much more relevant to GP practice.

4.       Each module will be conducted in 2 phases – workshop first followed by lecture sometimes later. In between, the participants may start scanning in their own practice. If they face any problems or uncertainties, they may bring   them up when we meet for the lecture.

  1. Non-alcoholic fatty liver disease (NAFLD) is a common disorder and refers to a group of conditions where there is an accumulation of excess fat in the liver. 
  2. The global prevalence of NAFLD is currently estimated to be 24%1. Community surveys utilizing ultrasonography or proton NMR spectroscopy have assessed the prevalence of NAFLD across geographical locales. 
  3. A study has shown that following a Mediterranean diet (rich in monosaturated fatty acids) can be beneficial for NAFLD patients.
  4. The field of NAFLD continues to evolve rapidly. Advances in the understanding of classical steatosis and disease progression are also reviewed to provide translational insights into how this knowledge can be used to prevent or treat the disease in the future.

Inability To Penetrate Vagina During Sexual Intercourse

 

A 28-year-old female, Indonesia Chinese housewife and married for 7 years with no children. She had never done sexual intercourse (SI) since the first night of the marriage. She felt strange and ashamed because of this failure. She has tried to relax her mind and allowed her husband to force while inserting the penis, but it still failed. This causes the painful feel in the vagina. She admitted that during the activities, she had set the positive thinking and felt to relax, but it had never happened.

 

She went to see the obstetrician after the come back from the honey moon. The doctors found no abnormality during routine physical and laboratory examination. She had fear when the doctor performed the vaginal examination. She was referred to the psychiatrist and had the same suggestions by the psychiatrist, i.e. to relax. There was also an unresolved conflict noted during the consultation.

 

In the second year of her marriage, while do all the suggestions from the professional, the sexual intercourse has never happened and increases the feeling of anxious and depressed to the condition. The marriage problem occurred because she felt that her marriage was never consummated. Despite having normal desire and feel to arouse while have a foreplay, she persistently refused sexual intercourse cause she knew how will be it ended and cause deeper feeling of failure.

Informed Consent in Elderly

Summary

Malaysia is rapidly aging and in fact Perak is the oldest state in Malaysia. According to the Statistics Department Perak will become an “aged” state in 2020 when its proportion of older population reaches 15%.  Prevalence of colorectal cancer increase with age. As the population ages, it will become a common problem often warranting surgical procedures requiring informed consent. Informed consent is integral to medical practice which involves authorization of a procedure based on the understanding of the procedure and not to be control by others.

Informed consent is not simply getting the patient's signature, but a therapeutic communication giving information about the nature, purpose and potential risks and benefits involved with and without the procedure.  Allowing a greater patient involvement in the decision making process is fundamental. 

Three domains of informed consent are freedom to decide, giving clear information, and decision-making capacity. Many elderly, including those with early dementia retain the capacity to give informed consent but should be sensitive to cultural, ethnic and religious values which vary from person to person. Joint decision-making approaches including friends and family are often recommended.

Patients with cognitive impairments have impaired ability to understand treatment options. Evaluating the capacity of patients with cognitive impairment is vital for a valid informed consent. Comprehensive Geriatric Assessment will help in determining the capacity however, these formal tests do not replace clinical history and examination.

Patients with cognitive impairment at early stage have the capacity to consent for themselves, while patients with various degrees of cognitive impairment the doctor should facilitate decision-making processes and patients incapable of making decision should the legal guardian take over at the best interest of the patient.

Documentation of the informed consent process in medical records is essential, detailing the act on the patient's best interest.

The first international symposium that is going to be held in Kuala Lumpur this year in 2020 is the leading of its kind in bringing closer the literature and science into a specific meeting. The Malay and Bahasa Indonesia language will be used exclusively in this meeting as a medium of communication. To be held at Universiti Kebangsaan Malaysia Medical Center Complex, Kuala Lumpur, Malaysia, uniquely, this meeting will specifically address sex as a pivotal topic with being respectful and mindful supports the good relationship between sex and marriage. How is our course difference from others?

Equitable, affordable, safe and efficacious health products is a global priority to be addressed under the proposed WHO Sustainable Development Goals.

The global need for blood safety was first addressed by WHO through the passing of the resolution in 1975, WHA 28.72 on Utilization and supply of human blood and blood products. Since then many additional resolutions and initiatives have been undertaken to develop and strengthen blood transfusion worldwide. In 2013, blood products were included into the Model List of Essential Medicines, reinforcing the need for governments to emphasize blood transfusion as part of their overall health development programme.

Despite the many proposed initiatives, universal access to quality and safe blood and blood components remain a challenge in many countries. To move the agenda further, WHO had developed a new Action Framework to Advance Universal Access to Quality and Safe Blood and Blood Components for Transfusion and Plasma Derived Medicinal Products, This framework is proposed to address the current barriers to safe blood, through a number of actions focusing on five strategic objectives.

In line with this new framework, we are proposing a seminar and workshop to disseminate information and gauge the current extent of its application within the blood services of the South and East Asian regions.

The objectives of the seminar are;

  1. Create awareness of the draft framework among stakeholders within this region
  2. Gather feedback on its feasibility and implementation with emphasis on local settings and regulation
  3. Share experiences and data in the areas of haemoviligance and donor vigilance within the region

 

The projected outcomes of this workshop should include;

 

  1. Closer collaboration and networking in this region among stakeholders, local agencies and international organisations such as WHO and ISBT
  2. Visualization of the extent of challenges within this region with regards to access to quality and safe blood and blood products
  3. Develop action plans using the proposed framework as guidance for further developing the transfusion services in this region
  4. Create trainees and transfusion personnel who are cognizant of the intricacies involved in the establishment of an effective national blood services

 

 

 

Proposed date and venue

The seminar/workshop is planned for for 1 ½ days on 4-5 March 2020, as a Pre-congress event prior to the Malaysian National Transfusion Medicine Congress which will be held on 6-8 March 2010 in the city of Malacca, Malaysia.

 

Facilitators and participants

Dr. Yuyun SM Soedermono, from WHO has been identified as a key facilitator for the workshop. We also expect to invite transfusion professionals from this region who will contribute to the seminar/workshop, which should include representatives from Singapore (?Dr. Ang Ai Leen), Indonesia (?Dr. Ni Ken Ritchie), Philippines (TBD), Thailand (TBD), Vietnam (TBD), Myanmar (?Dr. Thinda Aung), Sri Lanka and New Zealand (?Dr. Peter Flanagan) among others. Invitation has however not been extended yet until funding has been secured.

The Malaysian National Transfusion Medicine Congress attracts about 300 local participants and less than 50 international participants. We expect the turnout for this seminar/workshop to be about 80-120 participants, composed mainly of transfusion medicine specialists, haematologists and laboratory professionals.

 

Funding

ISBT support is requested for flight and accommodation of seven invited representatives from this region. The Malaysian Blood Transfusion Society will cover the cost of conference facilities and meals. We will also approach the local WHO office for assistance of this programme.

Cardiac contractility modulation (CCM) signals are non-excitatory electrical signals delivered during the cardiac absolute refractory period that enhance the strength of cardiac muscular contraction. The FIX-HF-5 study was a prospective randomized study comparing CCM plus optimal medical therapy (OMT) to OMT alone that included 428 New York Heart Association (NYHA) functional class III or IV heart failure patients with ejection fraction (EF) ≤45% according to core laboratory assessment. The study met its primary safety end point, but did not reach its primary efficacy end point: a responders analysis of changes in ventilatory anaerobic threshold (VAT).

Cardiac arrest is a treatable medical event that requires immediate action to save a life. Cardiac arrest can strike indiscriminately and without warning, often leaving grieving family members and friends to question what could have been done differently. 

When the heart stops, the brain will begin to die within 4 minutes. An ambulance takes 10-15 minutes to reach the victim. If CPR were instituted early, even before the arrival of the ambulance, it could make a difference to the victim’s survival. As cardiac (heart) arrest could occur at home, in the office, on the streets, bystanders able to perform CPR becomes valuable for the victim.

Institut Jantung Negara (IJN), being the premier cardiac hospital, have taken the first step of public responsibility by offering Basic Life Support (BLS) training. Basic Life Support (BLS) is the foundation for saving lives after cardiac arrest. It is designed for healthcare professionals as well as public to provide emergency care to patients in a wide variety of settings, and teaches both single-rescuer and team basic life support skills for application in pre-hospital and in-facility settings.

Brief description based on the Event Title

This program comprises a prepartory teaching course and mock exam for aspiring candidates of the MRCP PACES examination. It is is aimed at providing participants with useful guidance and tips in their preparation for the actual exam. The mock exam aims to simulate an actual examination situation to allow candidates to have a first hand experience on the running of the exam, followed by constructive feedback aimed to help the candidate polish his/her skills for the actual examination.
