Please find attached two letters from the Ministry of Health, for you kind attention.

1. Arahan YBMK Bil 1 Tahun 2020 – Self Declaration Kemudahan dan Perkhidmatan Screening dan Triaging (S & T) bagi COVID-19 di Klinik Perubatan Swasta, Pusat Jagaan Ambulatori Swasta dan Hospital Swasta yang Berdaftar atau Berlesen di bawah Akta 586, and;
2. Arahan YBMK Bil 2 Tahun 2020 – Self Declaration bagi Menyediakan Health Screening Booth (HSB) oleh Klinik Perubatan Swasta yang Berdaftar di bawah Akta 586

Please be informed that the private healthcare facilities stated above may fill the self-declaration form as attached and submit to CKAPS if they would like to conduct:

1. Screening & Triaging for COVID-19
2. Health Screening Booth including for COVID-19 (Private Medical Clinics Only – GP/Specialist)

By submitting the self-declaration forms, it means that the provider is acknowledging that he or she has read and understood all the SOP’s and will be responsible to ensure compliance to all relevant requirements throughout the entire process.
